I'd have to concur with this. It comes down to if the gal is actually enjoying it and getting off to it. I could really care less what the two dudes are doing at this point other than servicing her. We're here to see the woman in the throes of ecstasy, right?
I guess I'm also not into seeing humiliation or rough play where she's a syllable away from busting out the safe word- if they're even using one on set. If any of the participants aren't enjoying the scene- why are we watching that train wreck happening then? I'm not against some initial discomfort as some orifices can take some getting used to and the intensity of the moment can be overwhelming, but constant discomfort? No thanks.
I think like with many other sexual acts performed on film, some performances can be better than others. The ones that work well are the ones where all three people are comfortable and moving with one another in sync. What I've found non-erotic is when one of the men is there just occupying space and no movement whatsoever; almost a place holder to make it a DP. What can make the scene work for me is when all three people are in on the enjoyment, moving together and heat of the moment.
The other aspect of what makes a scene hot is what happens behind the scenes- does the cameraperson know what they're shooting? Is it in focus? Toss in some decent directing and editing skills and it can be hot. Sadly not all shoots have those components. I'd even dare say that with clever editing and skills behind the camera- a mediocre performance can be polished up a bit to being acceptable.
